Cobbs Garden Surgery is part of a Primary Care Network
Like all NHS GP practices across the UK, Cobbs Garden Surgery has become part of a Primary Care Network (PCN).
Our network is named CROWN and the practices we are clustered with are Red House Surgery and Whaddon Healthcare.
PCNs enable us to employ new additional staff and share resources with other practices. New staff include social prescribers, first contact physiotherapists and clinical pharmacists, and more staff will be joining us in the next few years.
The PCN allows us to work together with the community, mental health, social care, pharmacists, hospital and voluntary services. It allows us to enable greater provision of proactive, personalised and integrated care.
Where emerging primary care networks are in place in parts of the country, there are clear benefits for patients and clinicians.
Primary care networks are based on GP registered lists. They are small enough to provide the personal care valued by both patients and GPs, but large enough to have impact and economies of scale through better collaboration between practices and others in the local health and social care system.
